The Policy Language tools are for settings that want to just update their existing policy manual or add policy that is aligned with HCBS Final Rule to their policy manual.

The Remediation Plan tools are for settings that are not compliant in one or more of the HCBS Federal Requirements after the self-assessment review. These Remediation Plan tools will assist providers to create a remediation plan.

The Program/Service Plan tools are for settings that need assistance to update their current program design or service plan to align with the HCBS Final Rule requirements.

The Person Centered Plan Template tool is for settings to use and create a person-centered service plan for their clients.

The Self Assessment Plan tool is for providers to use and do a self assessment of their setting to determine whether their setting is in compliance with the HCBS Final Rule requirements.

The Bundle Paks are a collection of all the individual tools contained in either the the Policy Language Tool or the Remediation Plan Tool.
