HCBS Final Rule Person Centered Service Plan Template Tool

THIS PRODUCT APPLIES TO BOTH RESIDENTIAL AND NONRESIDENTIAL SETTINGS

CMS specifies that service planning for participants in Medicaid HCBS programs under Section 1915(i) of the Act must be developed through a person-centered planning process that addresses health and long-term services and support needs in a manner that reflects individual preferences and goals. This tool is designed to assist providers achieve this requirement.

This tool includes the following:

•  Person-Centered Service Plan template tool,
• Risk Management and Safeguards form,
• Self Directed Services form, and
• Modifications form.

This tool is designed to assist providers prepare to meet the requirements for the Medicaid HCBS program.

Most states have their state specific forms and/or procedures to submit remediation plans to agencies. This tool does not include any such state specific forms and/or procedures.

This tool will be delivered in a form fillable pdf format as a zipped file.
