• Remediation Plan & Validation tool aligned with HCBS Final Rule Federal Requirement #2,
• Remediation Plan & Validation tool aligned with HCBS Final Rule Federal Requirement #3,
• Remediation Plan & Validation tool aligned with HCBS Final Rule Federal Requirement #4, and

This tool is designed to assist providers prepare a remediation plan for their nonresidential setting to become aligned with HCBS Final Rule Federal Requirements. It will be delivered as a zipped file with all the tools above included in the zipped file.

Most states have their state specific forms and/or procedures to submit documents to agencies. This tool does not include any such state specific forms and/or procedures.
